Job Title: Director of Palliative Care Services Reports To: Chief Executive Office Location: Regional (State of Montana)

Suggested Pay: $110,000 - $140,000 

Position Overview 

The Director of Palliative Care will provide visionary leadership in designing, implementing, and expanding palliative care services across new markets and communities. This role combines strategic program development, operational excellence, and clinical leadership to ensure compassionate, person-centered palliative care that aligns with organizational goals and meets community needs. 

Key Responsibilities 

  • Program Development and Expansion 

  • Lead the design, implementation, and growth of palliative care programs in new markets. 

  • Conduct community needs assessments to identify service gaps, referral patterns, and partnership opportunities. 

  • Recruit, interview, hire and orient new staff members to support the launch and growth of agency palliative programs. 

  • Develop scalable program models that integrate with existing health systems, home-based care, and community resources. 

  • Establish key performance indicators (KPIs) and evaluation frameworks for new and emerging programs. 

  • Strategic Leadership and Integration 

  • Collaborate with senior leadership to develop strategic plans for expanding palliative services within the organization’s broader continuum of care. 

  • Build strong working relationships with hospitals, health systems, payers, and community-based organizations. 

  • Ensure program alignment with regulatory and reimbursement standards, including CMS, Joint Commission, and state-level requirements. 

  • Clinical and Operational Oversight 

  • Oversee clinical quality, patient outcomes, and experience across all programs. 

  • Partner with medical directors, nursing leaders, and interdisciplinary teams to implement best practices in serious illness care. 

  • Develop and monitor program budgets, staffing models, and resource allocation to ensure sustainability and growth. 

  • Education and Advocacy 

  • Serve as a thought leader and advocate for palliative care in emerging markets. 

  • Provide education and mentoring to interdisciplinary teams, community physicians, and healthcare partners. 

  • Represent the organization in public forums, conferences, and policy discussions related to palliative and serious illness care. 

Qualifications:

  • Bachelor’s Degree required 

  • Master’s degree in Nursing, Healthcare Administration, Public Health, or related field required; clinical background (MD/DO, NP, RN, LCSW) preferred. 

  • Minimum 7–10 years of progressive leadership experience in palliative care, hospice, or population health. 

  • Proven success developing and launching healthcare programs across multiple regions or markets. 

  • Strong strategic planning, partnership-building, and change management skills. 

  • Exceptional communication, leadership, and team-building abilities. 

Preferred Attributes: 

  • Experience in value-based care or alternative payment models. 

  • Characteristics including but not limited to, compassion, drive, and a commitment to growth and success. 

  • Exceptional relationship building skills with experience in community outreach, engagement and partnership creation and management. 

  • Demonstrated success scaling community-based programs. 

  • Understanding of diverse cultural and rural healthcare landscapes.
