Position summary

The Emergency Medicine Clinical Pharmacist provides direct clinical pharmacy services in the emergency department, optimizing safe and effective medication use for acutely and critically ill patients. This role collaborates closely with physicians, nurses, and care teams to refine pharmacotherapy plans, support emergency responses, and improve patient outcomes while maintaining regulatory and policy compliance.

Key responsibilities
  • Provide clinical pharmacy support within the Emergency Department, including real-time medication review and order verification.

  • Review, verify, and optimize medication therapy for emergency and critical care patients, incorporating pharmacokinetics and evidence-based guidelines.

  • Participate in emergency responses (e.g., codes, strokes, sepsis, trauma) and contribute to rapid clinical decision-making at the bedside.

  • Collaborate with physicians and nursing staff on medication selection, dosing, administration routes, and monitoring for safety and efficacy.

  • Ensure compliance with hospital policies, regulatory standards, and best-practice protocols for medication safety and stewardship.

  • Support medication education and in-services for physicians, nurses, and other clinical staff as needed.

  • Maintain accurate clinical documentation, interventions, and outcomes reporting in the medical record and pharmacy systems.

Required qualifications & skills
  • Active or Delaware-eligible Pharmacist license.

  • Bachelor's Degree in Pharmacy (PharmD or equivalent training strongly preferred in many clinical roles).

  • Board of Pharmacy Specialties (BPS) certification (e.g., Emergency Medicine Pharmacy BCEMP or Critical Care Pharmacy BCCCP).

  • Certificate or formal training in Emergency Medicine or Critical Care pharmacy practice.

  • Minimum 2 years of hospital pharmacy experience with direct clinical responsibilities, preferably in ED or ICU settings.

  • Completion of formal clinical pharmacy training such as residency or equivalent experience.

  • Current BLS, ACLS, and PALS certifications.

  • Strong clinical judgment, rapid decision-making skills, and effective interdisciplinary communication.

Core competencies
  • Emergency and critical care pharmacotherapy, including high-risk and time-sensitive medications.

  • Medication safety, reconciliation, and optimization in fast-paced ED environments.

  • Rapid clinical decision-making during resuscitations, sepsis bundles, stroke, and cardiac emergencies.

  • Interdisciplinary collaboration with emergency physicians, nurses, and other specialists.

  • Regulatory, policy, and formulary compliance aligned with institutional and national standards.

  • Clear, concise clinical documentation and communication of recommendations.

  • Patient-centered care, including counseling when appropriate in the ED setting
