- Community health organization operating across the Pee Dee region of South Carolina
- Collaborative provider team focused on improving patient care and outcomes
- Follows a Value Based Care model, with emphasis on measures like hypertension and diabetes control, cancer screenings, and cardiovascular care
- Opportunity to serve as collaborating physician for nurse practitioners within the organization

- Full-time, MondayFriday, 8:00 AM5:00 PM
- Provides primary care including histories, exams, lab tests, prescriptions, and referrals
- Performs timely EHR documentation within 48 hours of each visit
- Provides patient education, counseling, and shared decision-making around diagnosis, treatment, and prognosis
- Coordinates care with internal and external providers and hospitals
- Prioritizes Value Based Care metrics at patient visits
- Attends site, department, and provider meetings
- May serve as collaborating physician for nurse practitioners, per collaborative practice agreement
- Completes medical chart reviews to evaluate standards of care and treatment outcomes
- May perform specialty services after additional training, credentialing, and certification
- MD or DO from an accredited medical school
- Board Certified in field of practice
- Valid South Carolina medical license
- 13 years of experience preferred
- Ability to work in a fast-paced environment and manage stressful situations
- Small-town community in the Pee Dee region
- Close proximity to Hartsville's attractions, including Kalmia Gardens, Prestwood Lake, and Neptune Island Waterpark
- About 30 minutes from Cheraw State Park, offering scenic hiking trails and a lake
