You’ll step into an outpatient primary care role focused on preventive medicine, chronic disease management, and comprehensive patient care. You’ll work alongside a collaborative multidisciplinary team with integrated behavioral health resources and a focus on delivering quality care to underserved communities.

What makes you a great fit• You are a Certified Family Nurse Practitioner (FNP)• You hold an active New Hampshire APRN license or are eligible to obtain one• You have DEA registration or are eligible to obtain it• You have outpatient primary care experience• You’re comfortable diagnosing and managing acute and chronic conditions• You have strong independent clinical decision-making skills• You value preventive medicine, patient education, and continuity of care• You enjoy working in a collaborative, patient-centered environment• Previous FQHC or community health experience is a plus
