Job Description

POSITION TITLE: Staff Physician with Gender-Affirming Care (GAC)

Full or part-time 0.5-1.00 FTE

REPORTS TO (TITLE): Medical Director and Team Clinician Lead- Lombardi campus

SALARY RANGE: Physician: $211,660 - $255,709 DOE (Based on 1.00 FTE)

Join Us! United in heart, health, and justice, Santa Rosa Community Health (SRCH) provides high-quality, culturally responsive health care that meets the diverse needs of our community. Every day, we open our doors to care for patients in a way that honors them and meets their health care needs. We welcome everyone with compassion and respect for their culture, language, identity, and values-no matter what. Now in our 28th year, over 40,000 children, youth, families, adults, and elders rely on SRCH for the medical, dental, and mental health care and wrap-around services they need to get and stay well.

The ideal SRCH clinician is drawn to broad-based, full-spectrum primary care, including pediatrics, adult and geriatric care. SRCH prides itself on being a regional leader in preventive care services, as we regularly score in the top bracket of our managed Medi-Cal (Partnership Health Plan) quality improvement measures.

Lombardi campus - located in the original site when SRCH first opened its doors in 1996, Lombardi Campus is deeply embedded in a Spanish-speaking and largely immigrant community in the Roseland neighborhood in southwest Santa Rosa.

This campus is ideal for a clinician who is particularly skilled and drawn to work with a Latino population and thrives in a fully bilingual environment.

Lombardi is also the original site for our FNP Residency; as such, supporting and working in partnership with FNP colleagues is a philosophy deeply embedded at the site.

The position includes a focus on Gender-Affirming Care (GAC); you will be part of a small team of other clinicians who specialize in this care. A significant portion of your patient panel will be patients who identify as gender-diverse; patients represent a wide range of backgrounds, and many of whom are Spanish monolingual. Care of these patients includes, but is not limited to, counseling regarding options for Gender-Affirming Care; initiation and management of gender-affirming hormone therapy; mental health support (often in concert with in-house behavioral health and mental health support); and referrals to local specialty resources for treatments such as hair removal, speech therapy, and a range of gender-affirming surgeries. This clinic includes treatment of pediatric patients, though GAC for pediatric patients is not a requirement for this position.

As a focused provider of gender-affirming care, you will also see patients referred from both Santa Rosa Community Health campuses and outside clinical organizations (Sutter Health, other local FQHCs, Providence Health, et al). You may be asked to consult on management for SRCH patients seen in other clinics who may not need to be seen in Gender Clinic.

Dedicated administrative time is provided for management of Gender Clinic-related tasks, including coordinating with specialists regarding referrals; medication prior authorizations (in collaboration with administrative support staff); responding to referral denials; and other aspects of care coordination.

Additionally, you will have an opportunity to precept Residents- Family Medicine (MD/DO) or FNP - in your designated GAC clinic.
