Join Our Team as a Family Practice Nurse Practitioner
Are you a compassionate and dedicated Family Practice Nurse Practitioner seeking an impactful role that combines telehealth and home health visits? We are looking for a highly motivated professional to deliver exceptional patient care in a dynamic, supportive environment. This position offers the opportunity to make a meaningful difference in patients' lives across Milwaukee, with flexible scheduling and a focus on comprehensive healthcare delivery.
In this role, you will conduct annual risk assessments and chronic condition reviews for both pediatric and adult patients. The position is assessment-only, with no treatment or prescribing required. You will be responsible for scheduling and conducting approximately 12-14 telehealth visits per day or 6-7 in-home visits, with a flexible, mixed approach averaging around 10 visits daily. Home health visits are within a 30-mile radius of the provided address. The role involves scheduled days Monday through Friday, 8 am to 5 pm, with a one-hour lunch break, and equipment will be provided to facilitate your work.
This full-time position offers a competitive hourly pay rate of $95.48, paid weekly via direct deposit, with benefits including comprehensive medical coverage, paid sick leave, licensure, certification, travel reimbursements (when applicable), and a robust referral bonus program. You will receive initial paid orientation and ongoing education modules to support your professional growth, with flexible work hours within your scheduled shifts.
Candidates must be enrolled and credentialed in Medicard, possess an active Michigan medical license, and be comfortable seeing both pediatric and adult patients. A positive attitude, excellent communication skills, and ability to work independently are essential. Prior experience in telehealth and home health is preferred but not required.
