JOB SUMMARY

Family Service Coordinators provide case management/service coordination for children ages zero to three who evidence a developmental delay or other atypical development. Service Coordinators conduct screenings, facilitate eligibility determination, organize and participate in evaluations and develop plans for care. Service Coordinators link families with community resources, teach families how to be effective advocates for their children, and assist families in arranging for services their children may need at the time of discharge.

ESSENTIAL FUNCTIONS

  • Provide service coordination for children ages zero to three who evidence a developmental delay or other atypical development
  • Conduct developmental screenings and discuss and share results and next steps with families
  • Collaborate with team to coordinate evaluations/assessments, participate in assessments/evaluations and eligibility determinations, develop Individualized Family Service Plan (IFSP) with family and team, and ensure timely services are provided to families
  • Coordinate and facilitate periodic reviews of the IFSP and other necessary updates
  • Complete monthly contacts with all families on caseload
  • Link families with community resources and educate families how to be effective advocates for their children
  • Meet all productivity and accountability benchmarks for the program
  • Transition planning with families to ensure children are set up with any necessary services upon discharge
  • Ensure timely documentation and billing in EMR systems to accurately reflect services and contacts provided for families
  • Must be willing to work in a variety of environments including a family's home or child's environment
  • Must be willing to drive in variety of weather conditions and to all areas of Milwaukee County
  • Other duties as assigned

SKILLS & QUALIFICATIONS

  • Bachelor's Degree in a human service-related
  • Availability to work full-time hours
  • Possess valid Wisconsin driver's license with a clean driving record, reliable transportation and car insurance.
  • Ability to manage a caseload of 45 or more families
  • Able to work independently on a broad variety of projects.
  • The ability to contribute to a team environment and/or independently, to provide excellent customer service.
  • Ability to organize, plan and execute tasks in an efficient and profession manner.
  • Strong written and verbal communication skills.
  • Strong computer skills, especially with the Microsoft suite: (Word, Excel, Outlook).

LICENSES & CERTIFICATIONS

  • Valid Wisconsin Driver's License
  • CPR/First Aid certification preferred
