FCCP LICSW/LMHC Clinician (Fee for service)

Family Care Community Partnership Program

Job summary:

Independently Licensed Clinician with the Family Care Community Partnership (FCCP) program will be responsible for performing initial diagnostic evaluations, construct clear clinical formulations, and map out/review ongoing care plans with client and FSCC staff input.

Work Schedule Demands:

  • Per Diem up to 10 hours per week unless determined or approved by the VP of Family Development
  • Fee for service
  • Will be required to provide home based services
  • Due to the nature of the job, this individual must be flexible with their schedule to accommodate the client population. Occasional evenings and /or weekend availability may be required.

Required Qualifications:

  • Must hold a master's degree in social work or related field
  • Must hold an unrestricted license to practice in the State of Rhode Island. (LICSW, LMFT or LMHC)
  • Must have excellent written and oral communication skills.
  • Provide/maintain all required immunizations and/or vaccinations
  • Complete all required background checks
  • Must hold a current driver's license and be able to show proof of insurance.
  • Bilingual Spanish a plus.

Key Responsibilities:

  • Approves required documentation in DCYF- Rhode Island Family Information System (RIFIS) This includes:
  • Day 30- Review and sign CANS, complete the family visit and Diagnosis and SED/DD Determination.
  • Day 60- Review and sign SNCD
  • Day 90- Review Family Plan and sign and continue every 90 days
  • Completes all written work, evaluations, reports, progress notes in a timely manner.
  • Oversee the FCCP records in accordance with CCAP policies and procedures and DCYF standards.
  • Assists FCCP supervisor with the development of treatment and services in accordance with DCYF billing categories.
  • Ensure all documents are in accordance with Medicaid and/or state department standards.
  • Other duties as assigned
