Full-Time Home Health LPN Kansas City, MOPermanent Position | Competitive Pay + Benefits

Connected Health Care, a leading recruitment and staffing firm, is partnering with a respected home health provider in Kansas City, Missouri to hire a compassionate and reliable Licensed Practical Nurse (LPN) for a full-time, permanent opportunity. This position is perfect for an LPN who thrives in a patient-focused environment and enjoys providing individualized care in the comfort of patients homes.

Position Overview

Full-time, permanent opportunity*
Flexible scheduling based on patient needs*
Deliver skilled nursing care in a home health setting*
Build strong relationships with patients and their families

Job Responsibilities

Provide direct patient care following individualized care plans*
Monitor patient conditions and communicate updates to the RN or physician*
Administer medications and treatments as prescribed*
Perform wound care and other skilled nursing procedures*
Educate patients and caregivers on treatment plans and health maintenance*
Maintain accurate and timely patient documentation*
Ensure compliance with all agency policies and healthcare regulations

Education & Certification Requirements

Graduate of an accredited Practical Nursing program*
Active LPN license in the state of Missouri
Current BLS certification
At least 1 year of nursing experience preferred (home health experience is a plus)*
Valid drivers license and reliable transportation required
