Full Time Physical Therapist Assistant Home Health- Near Kansas City, KS Connected Health Care, a premier healthcare recruitment and staffing firm, is partnering with a respected home health organization near Kansas City, KS to hire a Full Time Physical Therapist Assistant (PTA). This opportunity is ideal for a motivated PTA who is passionate about helping patients regain mobility, improve strength, and maintain independence in the comfort of their homes. The Physical Therapist Assistant will work under the direction of a licensed Physical Therapist to implement treatment plans and provide high-quality rehabilitative care to patients in a home health setting. This role offers the opportunity to build meaningful patient relationships while supporting recovery and improving overall quality of life.Position Overview The Physical Therapist Assistant will assist with the implementation of individualized therapy plans designed by the supervising Physical Therapist. The PTA will provide direct patient care, monitor patient progress, and document therapy services accurately using Matrix EMR. This position requires strong communication skills, clinical knowledge, and the ability to work independently in a home health environment.Key Responsibilities
- Provide physical therapy treatments according to the plan of care established by the supervising Physical Therapist*
- Assist patients with therapeutic exercises to improve mobility, strength, balance, and functional independence*
- Monitor patient progress and report changes in condition to the supervising Physical Therapist*
- Document patient visits, treatments, and progress using Matrix EMR*
- Educate patients and families on exercise programs and safety techniques*
- Support patient rehabilitation goals and promote recovery in the home setting*
- Maintain compliance with home health regulations and agency policies*
- Collaborate with interdisciplinary healthcare teams to coordinate patient care*
- Ensure accurate and timely documentation of therapy sessions
- Graduate of an accredited Physical Therapist Assistant program*
- Active Physical Therapist Assistant license in the state of Missouri*
- Current CPR or Basic Life Support certification preferred*
- Knowledge of home health therapy practices and documentation standards
- Minimum of 2 years of Physical Therapist Assistant experience required
- Home health experience preferred but not required*
- Experience with Matrix EMR preferred*
- Strong patient care, communication, and organizational skills*
- Ability to work independently in a home health environment
