Physical Therapist (PT) Permanent Opportunity | South of Sacramento, CA
Connected Health Care is partnering with a highly respected healthcare organization south of Sacramento, California, to hire a full-time Physical Therapist. This is an excellent opportunity to join a collaborative rehabilitation team while enjoying a welcoming community with easy access to Sacramento, the Delta region, and Northern California's outdoor recreation.

Responsibilities
- Evaluate patients and develop individualized treatment plans*
- Provide evidence-based physical therapy services for orthopedic, neurological, post-operative, and general rehabilitation patients*
- Monitor patient progress and modify treatment plans as needed*
- Educate patients and families on treatment goals, home exercise programs, and injury prevention*
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other interdisciplinary team members*
- Maintain accurate documentation in the electronic medical record*
- Ensure compliance with all regulatory, safety, and organizational standards
Qualifications
- Doctor of Physical Therapy (DPT) or Bachelor's/Master's Degree in Physical Therapy from an accredited program*
- Current California Physical Therapist License (or eligibility to obtain)*
- BLS Certification*
- Excellent communication and interpersonal skills*
- Acute care, outpatient, or inpatient rehabilitation experience preferred*
- New graduates are encouraged to apply (if applicable)
