OPEN POSITION: Physician - Geriatric Primary CareSCHEDULE: - Full-Time - 4-Day or 5-Day Schedule- Monday to Friday- No Evenings- No Weekends- No Holidays- Minimal On-Call- Flexible Options AvailableCOMPENSATION:- Negotiable Sign-On Bonus- $250,000 to $275,000 Base Salary, depending on experience- Quarterly Bonus Opportunities- Malpractice Insurance- Health / Dental / Vision Insurance- AD&D / STD / LTD Insurance- Paid Time Off- CME Allowance- License / DEA Fees Paid- 401k- Full Package NegotiableLOCATION: Youngstown, OhioCOMPANY PROFILE: This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 50 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, and transitional care.POSITION DESCRIPTION: The physician serves as the primary care provider for a dedicated panel of geriatric patients.Patients are located in long-term care facilities (ALFs / ILFs) and private homes (private home patients are optional). Physicians are assigned a convenient geographic area in which all patients are located.Physicians see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs, but typically see 40 to 60 patients per week.Dedicated schedulers put together daily schedules / routes for the providers to ensure efficiency and appropriate time for every visit.Specific duties of the role include, but are not limited to:- conduct health assessments- develop personalized care plans- evaluate patient conditions and responses to treatment- adjust treatment plans as necessary- perform minor procedures and wound care as required and as settings permit- educate patients and families on health management, disease prevention, etc.- provide guidance on lifestyle modifications and self-care practices- coordinate with nurse practitioners, nurses, and other healthcare professionalsThe practice uses a mobile-based EHR with AI functionality to reduce administrative time.On-call is phone triage only with very low volume and providers rotate the on-call responsibility, typically one (1) night per month.REQUIREMENTS:- Ohio medical license- DEA- Board Certified or Eligible, Internal / Family / Geriatric MedicineHOW TO APPLY:To apply for this position, please send your resume to Jenn Kunkel at jenn@chsrecruiting.com or call (773) 998-1272.Please visit www.chsrecruiting.com/jobs for a full list of available opportunities at CHS Recruiting.
