Connected Health Care is currently seeking a passionate and dedicated Full Time Social Worker to join the team at our partnered hospital in Payson, AZ. If you have a heart for serving others and a desire to make a difference in the lives of patients and their families, we want to hear from you!

Job Description:

  • Provide comprehensive social work services to patients and their families, including psychosocial assessments, counseling, resource referrals, and discharge planning.*
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.*
  • Advocate for patients’ rights and coordinate community resources to ensure continuity of care.*
  • Maintain accurate and thorough documentation in compliance with regulatory standards and hospital policies.*
  • Participate in ongoing professional development and training opportunities.

Education and Certification Requirements:

  • Master’s degree in Social Work from an accredited institution.*
  • Current licensure as a Social Worker in the state of Arizona.*
  • Minimum of 2 years of clinical social work experience preferred.*
  • BLS certification required.
