Oregon Medical Group, part of the Optum family of businesses, is seeking a Geriatrician (FM/IM) to join our Complex Care Clinic team in Springfield, OR. The Geriatrician in the Complex Care Clinic provides comprehensive, patient-centered care for older adults with multiple chronic conditions, functional impairments, cognitive disorders, and psychosocial complexity. This role emphasizes interdisciplinary collaboration, longitudinal care, care coordination, and shared decision-making to optimize health outcomes, quality of life, and independence for medically complex geriatric patients.

Position Highlights:
Clinical Care
- Provide comprehensive geriatric assessments across medical, functional, cognitive, psychological, and social domains
- Diagnose and manage complex, multi-morbid conditions common in older adults
- Lead care for patients with frailty, dementia, polypharmacy, falls, mobility issues, and chronic disease burden
- Conduct medication reconciliation and deprescribing with focus on safety and evidence-based practice
- Develop individualized, goal-concordant care plans with patients and caregivers
- Provide advance care planning, goals-of-care discussions, and palliative-oriented care when appropriate
- Coordinate transitions of care across inpatient, outpatient, skilled nursing, home health, and community settings
Interdisciplinary Team Collaboration
- Collaborate closely with nurses, social workers, pharmacists, care managers, therapists, and behavioral health providers
- Participate in interdisciplinary case conferences and care planning meetings
- Provide clinical consultation and leadership to team members managing complex patients
Care Coordination & Systems-Based Practice
- Collaborate with primary care providers and specialists to reduce fragmentation of care
- Utilize population health and care-management tools to proactively address high-risk patients
- Support initiatives to reduce avoidable hospitalizations and emergency department utilization

Required Qualifications:
- Graduate of four-year U.S. Medical School or equivalent. Residency completed in Family Medicine or Internal Medicine
- Completion of an accredited Geriatrics fellowship
- Unrestricted OR Physician License or ability to obtain prior to start date.
- Current OR DEA certificate required prior to start date.
- Current BLS
- BE/BC in Family or Interenal Medicine & Geriatric Medicine
- EMR Proficient
- Excellent time management skills
Compensation for this specialty generally ranges from $226,000 - $366,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
