Job Description

POSITION SUMMARY:

The Direct Services Case Manager is responsible for participating in the provision of direct case management services to socially and economically disadvantaged Tribal members eligible and participating in programs administered by Anishnaabek Community and Family services. The position is responsible for assisting families in the identification and development of plans for self-sufficiency, including but not limited to services such as welfare assistance, public assistance, day care, transportation, supportive services, employment and training and other programs administered by the Sault Tribe.

ESSENTIAL FUNCTIONS: (includes, but is not limited to, the following)

  • Interviews clients and assists with completing an application for ACFS direct service assistance.
  • Finalizes application process with client and provides recommendations for direct service assistance.
  • Interprets and explains program policy and obligations to client.
  • Identifies resources, develops plans, and works with various departments, committees and outside agencies to assist clients with housing, transportation, employment and training needs.
  • Maintains client and program files.
  • Prepares written reports for client files.
  • Assists with the development of program policies and procedures and plans 

ADDITIONAL RESPONSIBILITIES: (includes, but is not limited to, the following)

  • Completes home visits where applicant is home-bound. 

CONTACTS:

Immediate peers, peers in other departments, immediate supervisor/manager, managers in other departments, clients and outside vendor/service providers.

PHYSICAL REQUIREMENTS:

Position sedentary primarily sitting and lifting a maximum of 10 pounds. Physical factors include constant sitting, use of hearing, typing; frequent use of near/field of vision; occasional standing, walking, carrying/lifting, reaching and travel. Working conditions include occasional exposure to extreme heat/cold. Potential hazards include constant computer use, frequent client contact and equipment use and occasional exposure to moving mechanical parts.

REQUIREMENTS:

Education: Bachelor’s Degree in Social Work or Human Services related field required.

Experience: One year of experience working in Social Services field or related program preferred.

Certification/License: Will be required to undergo and successfully pass a background check. Must maintain a good moral character standing. Must comply with the Sault Tribe’s Drug-Free Workplace Policy which may include random drug tests.

Knowledge, Skills, and Abilities: Knowledge of Federal, State and Tribal laws, rules and regulations related to mandatory reporting preferred.  Knowledge of interviewing techniques and community resources required.  Ability to develop, monitor and modify applicant service goals required. Knowledge of human behavior and behavioral sciences including human growth and development, dynamics of interpersonal relationships and family dynamics required. Must be able to communicate effectively both orally and in writing. Knowledge of and ability to use word processing and spreadsheet and database software required. Must have excellent public relation/customer service skills and be able to exhibit a friendly and positive attitude. Must be able to work under pressure and handle difficult situations. Native American preferred.
