Fraser is seeking a Health Services Manager who will be responsible for planning, directing, coordinating, and supervising the delivery of all health services within Community Living. This role ensures that all health-related services support the promotion, maintenance, and restoration of each client's health and well-being. The Health Services Manager will also provide leadership, support, and mentorship to Community Living nursing staff.
Job Duties:
Management Responsibilities:
- Provide oversight and day to day guidance to staff and leadership to support COVID-19 cases, contact tracing and care coordination.
- Lead the development and implementation of medical policies and procedures to ensure appropriate quality assurance measures and documentation to meet 245D and all applicable regulations.
- Develop medical trainings as appropriate for the Community Living Division and provide oversight for quality delivery.
- Monitor staff and client care issues for the Community Living Division, communicating with Director of Community Living, and provide recommendations as necessary.
- Provide on-call support to clients and staff in Supervised Living, Supportive Living, and Home-Based on a rotational basis.
- Provide direct supervision to nursing staff.
RN, Medical Care & Administration Responsibilities:
- Maintain health of clients by ensuring all healthcare appointments are tracked and completed.
- Adhere to orders given by healthcare professionals; ensure Community Living staff follow orders also.
- Follow up with healthcare facilities on medical results, reports, and care.
- Monitor client vitals and corresponding protocols as needed.
- Ensure that medication is properly administered and managed. This includes observing and monitoring administration by new staff, tracking psychotropic medications, tracking accuracy of medication administration record, reviewing medication/treatment errors, discrepancies, and refusals.
Staff Training:
- Provide medical trainings as requested (i.e. medication administration, protocols, seizures, signs and symptoms of illness, body mechanics, first aid, infection control, etc.).
- Provide training on the use of medical equipment, devices, or adaptive technology.
- Support of ongoing development of medical policy and record keeping procedures.
Qualifications:
- Minnesota Registered Nurse licensure required.
- Valid driver's license is required.
- One to two years of management or supervisory experience required.
- Three to four years of experience working with families and effectively communicating with parents/guardians required.

Location, Schedule: & Pay:
Fraser Bloomington, with travel to other Fraser locations and some remote work
Full time, working 40 hours a week Monday through Friday from 8 AM - 4 PM or 9 AM - 5 PM with some on-call rotation
The starting pay range for this role is $94,000 to $100,00 per year.
