- Build relationships with healthcare and senior-care referral partners.
- Conduct consistent in-person visits to hospitals, SNFs, rehabilitation facilities, assisted living, memory care, physician offices, hospice and home health organizations.
- Develop relationships with discharge planners, case managers, social workers, nurses, senior-living professionals, and community leaders.
- Generate qualified private-pay home-care referrals.
- Schedule one-on-one meetings, presentations, and lunch-and-learns.
- Follow up consistently with referral sources and prospective clients.
- Identify new referral opportunities throughout South Bay.
- Track contacts, visits, referrals, and results in our CRM.
- Coordinate with our Intake Coordinator and operations team to help convert qualified referrals into successful starts of care.
- Meet weekly outreach and monthly business-development goals.
- Home care or home health
- Hospice
- Hospitals/discharge planning
- Skilled nursing and rehabilitation
- Assisted living or memory care
- Senior services
- Healthcare or medical sales
- 2+ years of healthcare business development, community outreach, referral development, or relationship-based sales experience preferred.
- Knowledge of the San Diego healthcare/senior-care community strongly preferred.
- Existing relationships with local healthcare referral sources are a major plus.
- Confident developing new relationships from the ground up.
- Strong communication, presentation, organization, and follow-up skills.
- Self-motivated and comfortable being accountable for measurable results.
- Comfortable using a CRM.
- Valid California driver's license, reliable transportation, and auto insurance required.
- $28–$32 per hour
- Approximately 20–25 hours per week
