Healthcare Consultant I Case Management Coordinator Location: Homestead, FL (Miami-Dade County)Schedule: Full-time, Monday Friday, 8 AM 5 PMWe are seeking a bilingual (Spanish/English) Case Management Coordinator to join our team. This role is fully remote with training conducted via Microsoft Teams (4 6 weeks). The consultant will coordinate care for Medicaid Long Term Care/Comprehensive Program members through telephonic and face-to-face assessments in homes, assisted living facilities, and nursing homes.Responsibilities:
- Assess, plan, and coordinate case management activities
- Collaborate with providers and community resources
- Support members with condition management, medication review, and prior authorizations
- Promote wellness through coaching and motivational interviewing
- Document and monitor care plans in compliance with guidelines
- Bachelor s degree in Social Work or related field (no nursing degrees)
- Case management experience required; long-term care preferred
- Fluent in Spanish and English (reading, writing, speaking)
- Strong communication, organizational, and computer skills
