Job Summary:
The Outreach Specialist is responsible for enrollment of prospective participants into the PACE program
Responsibilities:
Coordinate and execute outreach, PR and community engagement activities to ensure consistent achievement of monthly referral, home visit and enrollment goals.
Identify opportunities to collaborate with community organizations and execute against those opportunities in a thoughtful manner that reflects our mission and value proposition.
Establish and maintain best in class relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and other potential referral sources in the service area.
Develop and implement annual marketing plan to achieve monthly enrollment goals with MOEE Director.
Provide education to referral source in the community regarding services.
Demonstrate in-depth knowledge of all relevant components of MOEE Playbook.
Comply with all rules pertaining to allowable and prohibited marketing practices.
Must be willing to work a varied schedule that may include evening nights, weekends and overtime.
Complete all required documentation and generate reports in a timely and accurate manner, including in Salesforce, Athena and MS Office Suite.
