JOB SUMMARY:

The HEZ Outreach and Engagement Specialist will work to provide outreach opportunities for patients assigned to the Health Equity Zone panel and community residents who lack access to basic needs and information of services. Build and operate social media outlets to reach a wide range of community members in the Cranston area. Help educate residents on HEZ and HEZ projects, work with local partner organizations to outreach residents of Cranston and what is available to them. To increase patient penetration into the community social service agencies for a wide range of social service issues. To be involved in LIT meetings to report to team of ideas and progress.

WORK SCHEDULE DEMANDS:

  • Full time, 35 hours per week.
  • Due to the nature of the work and level of responsibility, this individual maintains a flexible schedule, which may go beyond a regular workweek.
  • Grant-funded with possibility for extension

KEY RESPONSIBILITIES:

  • Provide outreach/in-reach to residents of Cranston and surrounding areas. Provide education of services available in the Cranston area. Offer assistance to help set appointments for residents.
  • Create and outreach on social media platforms. Engage with stakeholders to help boost HEZ social media platforms and educate community of services available to them.
  • Attend meetings to share and obtain information for outreach platforms and to contribute with new ideas for HEZ program.
  • Provide structured resident education through letters, phone, face-to-face encounters, and internet platforms. Engage in follow-up conversations and offer assistance to individuals.
  • Distribute outreach materials to community members, partner organizations and businesses to build coverage option awareness.
  • Develop, discover, and attend community events in order to promote the HEZ program and opportunities.
  • Collaborate with various local organizations to build awareness of the HEZ program and build referral linkages.
  • Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
  • Accurately provide required reporting to track goal achievement and client satisfaction.
  • Assist in the development and implementation of organizational outreach and engagement initiatives.
  • Other duties as assigned.

Requirements

REQUIRED QUALIFICATIONS:

  • CHW certified or currently obtaining (preferred)
  • Must be able to travel with use of a personal vehicle and have proof of vehicle insurance
  • Must possess the ability to read and interpret documents
  • Must possess the ability to write routine reports and correspondence
  • Must possess the ability to speak effectively before groups and actively engage the public in outreach situations
  • Must possess the ability to interact respectfully with diverse cultural and socio-economic populations
  • Oral and written communication skills at a level typically acquired through completion of a bachelor's degree program in public health, health education, communications or related program
  • An equivalent combination of education and experience will be considered
  • Knowledge of the health and human services infrastructure
  • Ability to effectively develop and nurture relationships with a diverse group of stakeholders
  • Ability to work independently and coordinate multiple tasks
  • Ability to enthusiastically communicate with potential enrollees
  • Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications, and Microsoft operating system
