Pay Range: : $59.00/hour – $62.99/Hour on W2 (USD)Schedule: Sunday- Saturday- 10am-7pm Description: Questionnaire: 1. Email: 2. Phone number: 3. Do you have a valid Pharmacist license in the state where the job is located? Needs to be yes, and add screenshot of unincumbered license below 4. Is the license in good standing with no current *or prior* disciplinary actions? Needs to be yes 5. License expiration date: if expiring soon make sure they are renewing or in progress and notate here 6. Do you have a valid BLS/CPR certification from American Heart Association or American Red Cross: Must be yes. Pharmacists need to be BLS in-person certified. 7. Expiration date of BLS/CPR certification: Do not submit unless they have a future date 8. Do you have Immunization Certification through an accredited organization (for pharmacists ONLY) (i.e. APhA)? Needs to be yes 9. Are you open to floating to other stores in the area? Mileage reimbursement applies only to miles exceeding 50 miles per day. 10. Are you able to work a flexible schedule and maintain consistent availability?
