Job Description
Physical Therapist needed to provide one-on-one therapy care for patients in their home setting!
Primary Job Responsibilities:
- Assess a person s condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.
- Collaborate with interdisciplinary team and colleagues to ensure a person s needs are met.
- Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.
- Utilize a variety of therapeutic modalities to maximize care provided to clients
Requirements:
- Vermont PT licensure
- Home Health experience and OASIS experience a must as this candidate will case manage
- McKesson/Netsmart EMR experience strongly preferred
