Job Description

Home Care - RN Case Manager

Full Time | 40 hours per week | Day shift | Salaried

The Registered Nurse Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to our clients in their homes around Providence, Pawtucket, and East Bay Rhode Island. The RN Case Managerconducts day-to-day nursing activities, to include assessment and education, for our clients in a one-on-one setting in their home. The RN Case Manager for Saint Elizabeth Home Care is a part of a cohesive and collaborative interdisciplinary team with ample administrative support. Our RN Case Managers are equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.

Qualifications for an RN Case Manager:

  • Current unencumbered RN license in Rhode Island.
  • Minimum of one year nursing experience, home care experience preferred.
  • CPR certification.
  • Reliable transportation, current clean driver's license, automobile insurance.
