Job Description

Empath Home Health, a member of Empath Health is seeking a caring and motivated Home Health Community Liaison for our Manatee, FL location.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

What you'll Do

  • The Liaison will act as a consultant to our referral sources to educate them about Empath Health services, access to hospice care and delivery of end of life care.
  • You'll be an expert in end-of-life services, as well as adept at marketing and educating our professional community.
  • He/she would be expected to identify and facilitate opportunities for an increased Empath Health presence and collaborations which will improve access to home care and related programs.

Hours: 8:00am-5:00pm, Monday through Friday. Asked to attend occasional evening & weekend events.

What You'll need

  • Education and/or Experience: Bachelor's degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
  • Minimum 2 years outreach, professional relations and/or sales experience in Home Health
  • Excellent communications and interpersonal relationship skills, including public speaking experience.
  • Must have reliable transportation/insurance and be willing to travel throughout the county.
  • Ability to work unsupervised exercising a high degree of discretion and independent judgement.
