Position Title: Home Health Care Registered Nurse (RN)
Department: Home Health Services
Supervisor: Chief Home Health Officer
Status/Hours Per Week: Non-exempt / Full-time (30-40 hours/wk)
Pay Range: The expected base pay for the position is $36-$65.02 per hour and may be increased based on other factors, such as language, certifications, etc.
Primary Function:
The Upham's Home Health Services has been in operation since 1975 serving thousands of Boston residents. Upham's Community Care is the only Boston health center to have a home health care and visiting nurse program.
The Home Care Nurse conducts home visits to patients for evaluation, provides direct skilled nursing care, executes physician's orders as required, and is professionally responsible to the nursing supervisor.
Duties & Responsibilities:
- Completes initial evaluation visits, including history and physical exam.
- Regularly evaluates the patient's nursing needs.
- Develops nursing care plan and makes necessary revisions.
- Provides health services requiring substantial specialized nursing skills including appropriate preventative and rehabilitative nursing procedures.
- Utilizes other community agencies and resources to provide comprehensive patient care.
- Coordinates additional health services provided to patient.
- Discuss changes in the patients' condition and needs with the physician and other appropriate personnel.
- Supports the patients' family in meeting the physical, psychosocial and other health needs of the patient.
- Oversees work of Home Health Aides as required in the patient's home.
- Provides community consultation and health education when appropriate.
- Keeps accurate records of patient's progress.
- Has overall responsibility for maintaining their patient's records according to Home Health Services' protocol.
- Participates in the regular case review meetings.
- Participates in the weekend/holiday rotation.
- Must provide a minimum of 25-30 patient encounters per week for a full-time position.
