Home Health LPN
Guthrie, OK (and surrounding areas)Full-Time | Company Car Provided. Are you a compassionate LPN who wants to deliver real, one-on-one care instead of rushing between hospital rooms? Join Companion Healthcare as a Home Health LPN and help patients receive skilled nursing care in the comfort of their own homes — where they feel safest and most independent..In this role, you’ll build meaningful relationships with patients and families while making a genuine difference in their quality of life every single day. What You’ll Do
- Perform comprehensive assessments and document patient condition at every visit
- Monitor vital signs, review medications, and complete evaluations
- Administer physician-ordered medications safely and accurately
- Provide skilled wound care, dressing changes, and track healing progress
- Educate patients and families on home care techniques, medication management, and health strategies
- Collaborate with the care team to deliver personalized, high-quality support
- Graduate of an accredited nursing program
- Current Oklahoma LPN license (required)
- Current CPR certification (required)
- 2+ years of home health, palliative care, or related experience (preferred)
- Strong communication skills and a genuine passion for patient-centered care
- Valid driver’s license and willingness to travel to patient homes in the area
- Company car provided for all work-related travel
