We are hiring a Medical Social Worker (MSW) for a Home Health position providing skilled nursing care to patients in their homes in the Greensburg and surroundingareas. 

What You’ll Do: 

  • Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

  • Conduct comprehensive psychosocial assessments, identify patient and family needs, and develop individualized care plans that address emotional, social, and environmental factors 

  • Educate and support patients and families by connecting them with community resources, counseling services, and care planning guidance to ensure they feel informed, supported, and empowered 

  • Apply critical thinking and problem-solving skills to navigate complex situations, advocate for patient needs, and maintainaccurate, timely, and compliant EMR documentation

  • Work independently, but never alone

  • Travel to patient homes within assigned territory

​MSW Qualifications

  • Master’s in Social Work from graduate school accredited by the Council of Social Work

  • One (1) to two (2) years post graduate social work experience in a health care setting

  • Licensed MSW preferred

  • Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are .
