- Deliver direct patient care and case management for home health patients
- Develop, implement, and revise personalized care plans
- Monitor and manage patient medications
- Teach patients and caregivers self-care, medication, and dietary instructions
- Supervise Home Health Aides and ensure a safe care environment
- Maintain accurate clinical documentation and communicate with physicians and care teams
- Current RN or LPN license from the MA Board of Nursing
- Minimum 2 years nursing experience; home health experience preferred
- CPR Certification
- Strong communication, reliability, and compassion
- Job Types: Full-time, Part-time
- Locations: Natick, Walpole, and surrounding areas in Massachusetts
