Home Health Occupational Therapist

Full Time

Coverage area: Newaygo, Mecosta, Mason, Oceana Counties

What You'll Do:

  • Evaluate patients and develop individualized plans of care based on functional needs and goals

  • Perform initial and ongoing assessments in accordance with OASIS and regulatory guidelines

  • Deliver high-quality occupational therapy services in the home setting

  • Educate patients and caregivers on ADLs, adaptive equipment, and home safety

  • Collaborate with physicians and interdisciplinary team members to ensure continuity of care

  • Adjust care plans as needed and obtain physician orders for changes

  • Support discharge planning and long-term patient success

  • Provide guidance and supervision to COTAs and other clinical team members

What is Required?

  • Graduate of an accredited Occupational Therapy program

  • Current, unrestricted OT license valid for the state of work

  • Minimum one (1) year of experience in a clinical care setting

  • Able and willing to travel within branch/office coverage area.

  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.

  • Must be able and willing to travel 50%

  • Able to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
