Home Health Physical Therapist (PT)
Location: Mason & Jackson Counties, WVSchedule: Full-Time | 40-Hour Weekly Guarantee | Day Shift
  • Shift: Day shift
  • Weekly Hours: 40-hour guarantee
  • Mileage Reimbursement: Yes (between patient visits, from first patient to last patient)
Pay Rate: $49/hr - $52/hr 
Position Overview
We are seeking a compassionate and skilled Home Health Physical Therapist to join our team, serving patients throughout Mason and Jackson Counties in West Virginia. This role offers the opportunity to provide one-on-one care in patients’ homes, helping them regain independence and improve their quality of life.
Key Responsibilities
  • Develop and implement individualized Plans of Care (POC) for patients in their homes
  • Provide hands-on physical therapy treatments tailored to patient needs
  • Conduct comprehensive patient evaluations and ongoing assessments
  • Monitor and document patient progress, adjusting treatment plans as needed
  • Educate patients and caregivers on therapeutic exercises, safety, and home care strategies
Required Qualifications
  • Active Physical Therapist (PT) license in West Virginia or compact license
  • Strong clinical knowledge and ability to work independently in a home health setting
Preferred Skills
  • Excellent assessment and evaluation skills
  • Strong communication and interpersonal abilities
  • Ability to build rapport with patients and caregivers
Certification Requirements
  • Licensed Physical Therapist (PT) – West Virginia
