Job Description

Position Summary

The Home Health Physical Therapy Assistant provides treatment and follow-up care to patients referred for physical therapy under the direction and supervision of physical therapists and in accordance with the established plan of care. This role supports patients in achieving their highest level of function by discharge or discontinuation of services.

The Physical Therapy Assistant works collaboratively with physical therapists, physicians, and other members of the rehabilitation team to promote continuity of care and support the overall physical therapy and rehabilitation plan of care. The caseload may include patients ranging from newborns to geriatric populations.

Key Responsibilities

  • Provide direct patient care within the scope of licensure and according to the physical therapist's plan of care.

  • Monitor patients for changes in condition that may require reassessment by the physical therapist or physician.

  • Perform treatment interventions, collect objective data, and contribute to discharge planning.

  • Provide patient and caregiver education and training.

  • Use appropriate equipment and direct support staff assisting with patient care.

  • Follow established plans of care and demonstrate clinical competence in delivering treatment.

  • Track and ensure appropriate clinical supervision by the physical therapist for shared patients.

  • Document patient progress and communicate updates to the physical therapist.

  • Participate in monitoring patient achievements and provide appropriate written documentation.

  • Contribute to discharge planning, including coordination with other disciplines and identification of continuing care needs.

  • Participate in case conferences and team conferences.

  • Provide supervision and mentorship to aides, students, observers, and volunteers.

Minimum Qualifications

  • Graduate of an approved physical therapy assistant program with an Associate of Arts degree.

  • No clinical experience beyond educational requirements is required.

  • Must be able to perform all phases of the position after an initial three-month period.

  • Current Maryland Physical Therapy Assistant license, temporary Maryland license, or eligibility while awaiting state board examination or state reciprocity.

  • Current BLS (CPR) certification in compliance with organizational policy.

  • Strong problem-solving, decision-making, and critical-thinking skills.

  • Strong organizational and time-management skills.

  • Ability to learn and effectively use the medical center computer system and point-of-care documentation system.
