Registered Nurse (PRN) - Home Health

As a Home Health RN, you will:
Deliver skilled nursing care to patients in their homes in accordance with physician orders and the individualized plan of care
Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations
Develop, implement, and update personalized care plans based on patient needs and progress
Provide education and support to patients, families, and caregivers to promote independence and improve outcomes
Monitor patient conditions and communicate changes to physicians, Clinical Supervisors, and care team members
Coordinate care with an interdisciplinary team to ensure continuity and quality of care
Accurately document patient visits, assessments, and care provided in accordance with company standards and regulatory requirements
Ensure compliance with HIPAA and maintain strict patient confidentiality
Promote a safe environment by identifying and addressing potential risks in the home setting
What You'll Need
Current, unrestricted Registered Nurse (RN) license in the state of practice
Minimum of 1 year of clinical nursing experience (home health experience preferred)
Strong assessment, critical thinking, and communication skills
Ability to work independently and manage a patient caseload
Willingness and ability to travel within assigned territory (50%)
Valid driver's license, reliable transportation, and current auto insurance
Ability to lift/move up to 50 - 100 lbs and assist with patient mobility
Associate or bachelor's degree in nursing (BSN preferred)
Experience with HomeCare HomeBase (preferred)
