Job Description

Job Title: Home Health Registered Nurse-{168807} Location: Vienna, WV 26105

· Coverage Area: Wood, Wirt, Calhoun, Ritchie, Pleasants, and Tyler counties

Pay: $56.14/hour + $0.54/mile mileage reimbursement Schedule: Monday-Friday, 8am-5pm (No On-Call)

Overview: We are seeking a dedicated Home Health Registered Nurse (RN). This role focuses on delivering high-quality, patient-centered care in a home health setting. The RN will be responsible for comprehensive assessments, care coordination, and ensuring optimal patient outcomes across a multi-county coverage area.

Key Responsibilities:

Perform comprehensive assessments of high-risk patients in the home setting
Develop and implement individualized care plans in collaboration with primary care providers
Coordinate care with physicians, social workers, discharge planners, and other healthcare professionals
Manage care transitions and ensure appropriate, cost-effective levels of care
Identify and address non-medical barriers to care, including housing and transportation needs
Monitor patient progress and maintain accurate clinical documentation
Promote health education, illness prevention, and patient advocacy

Required Skills & Qualifications:

Active, unrestricted RN license (WV or compact license) (ASN or BSN)
Minimum 1 year of Home Health experience
OASIS experience required

Preferred Experience:

Experience with Homecare Homebase
Case Management certification
