 RN Case Manager (Marshall & Jackson County, AL)

 Hour Guarantee: 32 hours 

Productivity Expectations: 25 points = 40 hours; part of on‑call rotation with case managers 

Coverage Area: Marshall & Jackson Counties, Alabama 

Shift: Monday–Friday, 8:00 a.m.–4:30 p.m. 

Mileage: Reimbursed from first patient to last patient 

Position Overview

The RN Case Manager is responsible for managing a home‑based hospice caseload within Marshall and Jackson Counties. 

This role requires a highly skilled, independent hospice nurse with the ability to manage complex patient needs, complete documentation in Home Care Home Base/Pointcare, and participate in interdisciplinary care planning.

Key Responsibilities

• Hospice patient management across Marshall & Jackson Counties, ensuring comfort, safety, and symptom control

• Conduct admissions, recertifications, deaths, and discharges

• Complete daily documentation and maintain productivity expectations (up to six visits/day)

• Participate in IDG care team meetings and prepare all required notes

• Manage medication needs, supply ordering, and care coordination

• Work independently in diverse home environments across all socioeconomic backgrounds

• Serve in the backup on‑call rotation: one weeknight per week and one weekend per month (supporting the 7/7 primary on‑call teams

Required Qualifications

• Active RN license (Associate degree or higher)

• Previous home hospice experience — must be fully competent in managing hospice patients independently

• Home Care Home Base (HCHB) / Pointcare experience

• Strong clinical judgment and ability to work autonomously

• Experience with interdisciplinary care planning and IDG participation
