Home Health Registered Nurse (RN) – Summersville, WV
Weekly Hour Guarantee: 36 hours

Position Overview
We’re looking for a dedicated Home Health RN to join our Summersville, WV team. This role is ideal for someone who thrives in autonomous patient care, enjoys meaningful one‑on‑one interactions, and is confident managing a diverse home‑health caseload. You’ll work Days, Evenings, and some Weekends with a rotating on‑call schedule.
Responsibilities
- Complete comprehensive assessments, initiate care plans, and establish patient baselines.
- Evaluate patient status after hospitalization or interruption in services.
- Conduct timely reassessments to ensure continued eligibility and update care plans.
Required Qualifications
- Active RN License in the state of West Virginia
- Strong clinical judgment and ability to work independently in the field
- Reliable transportation
- Home health experience
