Job Description
Home Health RN
- Weekly Hour Guarantee: 32 hours
- On‑Call/Weekend Requirement: Yes — one weekend per month
- Coverage Areas: Shelbyville, KY, including Shelby County and portions of Eastern Jefferson County
- Shift Schedule: Monday–Friday, 8:00 AM–4:30 PM
Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient
Top Responsibilities: Provides skilled nursing services in the home setting, including start‑of‑care admissions, comprehensive assessments, and ongoing patient management in alignment with physician orders. Ensures accurate, timely documentation, collaborates with the interdisciplinary team, and supports high‑quality, compliant patient care delivery.
Required Qualifications/Experience:
Active RN licensure
Minimum 1 year of home health experience
HCHB EMR experience
Preferred Skills/Attributes:
OASIS certification (strongly preferred)
