• * Complete start‑of‑care assessments with accuracy, compassion, and confidence
• * Build trust with patients and families from the very first visit
• * Develop individualized care plans that set patients up for success
• * Coordinate with physicians, therapists, and the clinical team to ensure seamless transitions
• * Document efficiently using our supportive EMR system
• * Serve as a clinical resource and role model for the team
• * Bring positivity, professionalism, and a can‑do attitude to every day

Requirements

• * Active RN license
