Job Description
We are currently seeking an experienced and empathetic Hospice Admissions Nurse to serve as a vital first point of contact for patients and families beginning their hospice journey.
The Hospice Admissions Nurse is responsible for conducting thorough clinical assessments of patients referred to hospice care, ensuring timely and seamless admissions. You'll act as a clinical guide and emotional support for families, while collaborating with interdisciplinary teams to create personalized and effective care plans.
Location: Field based in Fairfield County, Connecticut:
This is a Full Time in-field position and will cover the Southern Territory of Fairfield County, Connecticut (Norwalk, Darien, Stamford, Greenwich and surrounding towns).
Key Responsibilities:
Admissions Process
Conduct clinical assessments and determine hospice eligibility.
Review medical records, complete admission documentation, and ensure regulatory compliance.
Educate patients and families about hospice philosophy and care plans.
Care Coordination
Facilitate smooth transitions from hospital, home, or healthcare settings to hospice.
Collaborate with the interdisciplinary team (nurses, social workers, chaplains, etc.).
Coordinate delivery of medications, equipment, and services.
Education & Advocacy
Provide ongoing education to families and referral sources on hospice services.
Advocate for patients' rights and end-of-life wishes.
Documentation & Compliance
Complete electronic medical records (EMR) accurately and in a timely manner.
Ensure adherence to HIPAA, Medicare, CT DPH, and CHAP guidelines.
Support hospice team with clinical triage and referral management tasks.
Qualifications
Bachelor of Science in Nursing (BSN) from an accredited institution required.
Registered Nurse (RN) license in the State of Connecticut or Compact license required (active and in good standing).
Minimum 5 years of nursing experience, with at least 2 years in hospice care.
Hospice and Palliative Care Nursing Certification (CHPN) preferred; required within 2 years of hire.
Strong communication, interpersonal, and organizational skills.
Proficient in EMR systems and clinical documentation.
Deep understanding of hospice regulations, Medicare CoPs, and CHAP standards.

Compensation:
Our target pay range for this position is $101,000 to $109,000 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
