As a Hospice RN Case Manager, you will be a vital part of a dedicated interdisciplinary team—bringing together medical professionals, caregivers, and spiritual counselors to create a deeply meaningful care experience tailored to each individual patient.

🩺 Primary Job Responsibilities

  • Assess Patient & Family Needs Determine the scope and frequency of services based on patient acuity and family support.
  • Provide Compassionate Clinical Care Address symptoms, disease progression, patient education, and compliance to ensure comfort, understanding, and quality of life.
  • Promote Quality of Life Help patients live fully within their limitations—honoring their goals, preferences, and dignity.
  • Support Patient & Family Understanding Maximize engagement with the care plan through teaching, communication, and emotional support.
  • Coordinate Care Seamlessly Ensure each patient receives comprehensive, efficient care through collaborative teamwork.
  • Lead & Supervise Care Team Members Establish care plans for CNA/HHA disciplines and provide ongoing supervisory support per agency guidelines.
  • Continue Growing Professionally Stay up‑to‑date through educational programs, conferences, in‑services, and ongoing nursing development.
  • Document with Excellence Complete accurate, timely documentation that reflects patient status, care provided, results, and pain levels.

Requirements

  • Naturally flexible and empathetic toward others
  • Graduate of an accredited school of nursing
  • Current RN license in good standing to practice in the Commonwealth of Pennsylvania
  • Minimum of one year experience in the homecare or a medical/surgical setting
  • Experience with pain and symptom management interventions
  • Understanding of hospice philosophy, principles of death/dying
  • Maintains clinical and professional competence
  • CPR certified
  • Driver's License and Car Insurance in good standing
  • Reliable vehicle
  • Possesses the ability to follow oral and written instructions
  • Possesses the ability to communicate effectively with all members of the care team
  • Possesses good organizational skills.
  • Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
  • Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
