Job Description
Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer flexible work options and outside of the box opportunities that focus on your needs and interests.Well established hospice agency has a need for an experienced Hospice RN Case Manager to see patients primarily in their homes and facilities in Greater Boston, MA area.Full-time, Monday-Friday, 40 hours/weekShorter works weeks consideredNo weekends or on-call required!Great pay rate plus mileage reimbursementPrimary nurse provides quality nursing care to identified Hospice patients. The scope of the primary Hospice RN includes direct patient care, coordination of patient and patient advocacy. Patient includes the individual, family/significant other and community. Must be comfortable with issues relative to end-of life care, death and dying.Requires minimum 1 year of Hospice RN experience with a Medicare certified agency.Great benefits include:
