What You'll Do

As a Hospice Social Worker, you'll play a vital role in supporting patients and families through emotional, social, and practical challenges that come with serious illness. You will:

  • Provide emotional and psychosocial support to patients and families in their homes, facilities, or care settings.
  • Conduct assessments and develop individualized care plans as part of the interdisciplinary team.
  • Guide families through end-of-life planning, grief resources, and community support options.
  • Advocate for patient wishes and help navigate complex systems, including insurance, advanced directives, and long-term care planning.
  • Participate in team meetings to coordinate holistic care with nurses, aides, chaplains, and volunteers.
  • Offer bereavement and grief support when needed.
