Care Initiatives is currently seeking a PRN Hospice Social Worker to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities.

What You'll Do & Key Responsibilities:
Assess patients' psychosocial needs and complete social historiesDocument care plans, progress, and follow-up per regulationsProvide counseling and emotional support to residents and familiesSupport patients and families with adjustment, rights, and orientationCoordinate referrals and act as a liaison with community resourcesPartner with the care team to enhance quality of life and outcomesPlan and support patient, family, and community engagement activitiesPromote facility programs and services to referral partners
What it takes to be a Social Worker at Care Initiatives Hospice:
Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and current, valid social work license (LMSW).Hospice or palliative care experience, preferred.Two (2) years of verifiable experience working as a Social Worker, preferred.Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
