Job Description

Guardian Hospice Care is seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will be responsible for providing support and assistance to individuals and families in need. Your role will involve conducting assessments, developing care plans, and connecting clients with appropriate resources and services. The ideal candidate will have experience in clinical research, crisis intervention, and behavioral health.

Responsibilities:- Conduct comprehensive assessments to identify the needs and strengths of clients- Develop individualized care plans based on assessment findings- Provide counseling and support to individuals and families- Connect clients with community resources, such as housing assistance, financial aid, and healthcare services- Collaborate with healthcare professionals to coordinate care for clients- Advocate for clients' rights and ensure they receive appropriate services- Monitor client progress and make adjustments to care plans as needed- Provide crisis intervention as necessary- Maintain accurate and up-to-date documentation of client interactions

Requirements:- Bachelor's or Master's degree in Social Work or related field- Valid state licensure or certification as a Social Worker- Experience working in a hospital, nursing home, or behavioral health setting is preferred- Knowledge of clinical research and evidence-based practices in social work- Familiarity with special education programs and services is a plus- Strong communication and interpersonal skills- Ability to work effectively in a team environment- Compassionate and empathetic approach to working with individuals in need
