What our employees say about Wellspring!Housing Case Manager & Social Services Coordinator Duties and Responsibilities-Assists clients in achieving their housing goals-Willingness to work with clients with serious mental illness-Develops a person-centered service plan for each client-Assesses each client's needs, with an emphasis on strengths and barriers related to getting & keeping their housing -Works with property management staff to help clients maintain their housing-Identifies strategies that will help clients maintain their housing and strengthen their recovery-Coordinates/facilitates speakers, groups or events that foster community, identify helpful resources, and reinforce wellbeing- Supports clients in understanding the basic expectations of being a tenant, lease requirements, household maintenance, etc.-Collaborates with landlords and other community partners, advocating for client needs and rights. -Assists clients in applying for rental assistance, SNAP benefits, social security income, etc. -Participates in regular supervision and team meetings
