SCOPE OF THIS RFP The State of New Hampshire needs an external evaluation partner to oversee their Rural Health Transformation Program (RHTP) — a CMS-funded statewide initiative. The vendor will evaluate five health programs running across New Hampshire, monitor performance data monthly, report findings to state leadership and CMS, and help course-correct when programs are not on track. This is an evaluation and advisory services contract — NOT a staffing placement contract. Contract value is up to $25 million through 2031.POSITION 2 — Implementation Science Specialist What they will do: Assess why health programs are or are not working using implementation science frameworks; support mid-course corrections Minimum experience needed:- 5+ years applying implementation science in healthcare or public health- Must know at least one major framework: EPIS, CFIR, or PRISM- Experience conducting implementation-focused evaluations NH or New England experience is a strong plusKEY NOTES FOR SOURCING- New Hampshire or New England experience should be flagged immediately across all roles — the RFP specifically asks about NH experience and it will strengthen our scoring- This is a proposal engagement — candidates are being proposed for a contract role pending award.
