ChildServe is seeking anIn-Home Medical Day CareProviderto provide personal care and supervision for an individual with special health care needs in their home.
ScheduleVarious hours between 9am – 5pm M-F
How You’ll Make an Impact
- Provide direct care and assist individuals with daily living and behavioral needs in their home
- Implement care plans and provide ongoing feedback regarding additional or changing care plan needs
- Perform duties related to the tracking and upkeep of participant’s personal belongings
- Responsible for timely and accurate communication regarding services with families, staff, and supervisors
- Transport individuals to outings in the community
What You’ll Need:
- Must be at least 18 years old to apply
- Experience working with or knowledge of individuals with special health care needs preferred
- Provide your own transportation
- Ability to electronically document services provided
- Ability to communicate effectively in reading, writing, and speaking the English language
- Ability to lift up to 35 lbs. be mobile within the assigned work area, and frequently bend, lift, reach, and stand
