Job Description

Incident Reporting & Risk Management CoordinatorPosition SummarySouth County Mental Health Center, Inc. is seeking a detail-oriented Incident Reporting & Risk Management Coordinator to support organizational safety, compliance, and risk management efforts. This position is responsible for investigating incidents, maintaining reporting systems, analyzing trends, ensuring regulatory compliance, and recommending corrective actions to promote a safe environment for clients, staff, and stakeholders.About UsSouth County Mental Health Center, Inc. is a private, non-profit, Joint Commission-accredited behavioral healthcare provider serving Southeast Palm Beach County. We are dedicated to delivering compassionate, high-quality mental health services to individuals and families in our community.Minimum Qualifications
  • Bachelor’s degree in Health Services Administration or related field
  • Minimum one (1) year of experience in health services
  • Strong written and verbal communication skills
  • Proficiency with Microsoft Office applications, including Word and Outlook
  • Strong analytical, critical thinking, and problem-solving abilities
  • Ability to maintain confidentiality and professionalism
Preferred Qualifications:
  • Master’s degree in Health Services Administration or related field
  • Administrative experience in healthcare settings
  • Experience with behavioral health or mental health services
  • Familiarity with healthcare incident reporting systems and regulatory requirements
Duties and ResponsibilitiesIncident Reporting & Documentation
  • Report incidents through the Agency for Health Care Administration (AHCA) Incident Reporting System (AIRS)
  • Submit reportable incidents to the Department of Children and Families (DCF) Incident Reporting and Analysis System (IRAS)
  • Thoroughly document incidents, including dates, times, locations, involved parties, and event details
  • Maintain accurate incident records and tracking logs
Investigation & Analysis
  • Investigate incidents to determine root causes and contributing factors
  • Analyze incident trends and identify opportunities for improvement
  • Develop recommendations to reduce future risks and enhance safety practices
  • Support corrective action planning and follow-up activities
Grievance & Compliance Management  
  • Assist with grievance investigations when required
  • Review completed grievances and maintain grievance logs
  • Ensure compliance with regulatory reporting requirements and organizational policies
  • Support audits, reviews, and regulatory inquiries as needed
Reporting & Quality Improvement
  • Prepare clear and concise incident reports and summaries
  • Present findings and trends to the Quality Assurance Committee
  • Utilize incident data to support risk management and quality improvement initiatives
  • Recommend process improvements to strengthen incident reporting procedures
Additional Duties
  • Participate in safety, compliance, and quality improvement activities
  • Perform other duties and special projects as assigned

  • Accuracy and timeliness of incident reporting

  • Compliance with reporting requirements and regulations
  • Effectiveness of risk reduction and quality improvement initiatives
  • Timeliness and accuracy of grievance tracking and documentation
  • Communication and collaboration with internal stakeholders
