Job Details

• Evaluate and assess patients with injuries, illnesses, or post-surgical conditions to determine functional limitations and rehabilitation needs
• Develop individualized treatment plans focused on improving mobility, strength, balance, and functional independence
• Provide hands-on therapeutic interventions including gait training, mobility training, therapeutic exercise, and manual therapy
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers to coordinate patient care
• Educate patients and families on recovery goals, home exercise programs, and injury prevention strategies
• Monitor patient progress and adjust treatment plans based on clinical outcomes and functional improvements
• Document evaluations, treatment plans, and patient progress accurately within the electronic medical record (EMR)
• Assist with discharge planning, including recommendations for home health, outpatient therapy, or rehabilitation facilities
• Ensure compliance with hospital policies, safety standards, and regulatory requirements
• Maintain patient safety during transfers, ambulation, and therapeutic activities
