- Conduct patient intake and collect required clinical and demographic information; ensure accurate and timely documentation in the EMR
- Coordinate and schedule home health visits based on established plans of care and clinician availability.
- Serve as a friendly point of contact for patients, families, referral sources, and clinicians
- Verify insurance eligibility and authorizations in collaboration with billing and administrative teams
- Support nursing staff with care coordination and documentation tasks
- Communicate with physicians regarding verbal orders, clarifications, and plan of care updates per agency protocol.
- Track Start of Care (SOC), Recertifications, and Discharges and follow up on missing documentation or physician orders.
- Maintain clear communication between clients, families, case workers, and healthcare providers
- Assist with patient education and smooth transitions into home health services
- Monitor patient records and ensure compliance with agency policies, Medicare, and HIPAA guidelines
- Participate in team meetings, case conferences, and care coordination discussions
- Provide high-quality, compassionate support to patients, families, and staff

- Minimum of 1 year of home health experience (Medicare-certified agency preferred).
- Strong knowledge of OASIS, 485s, Plan of Care, and CMS compliance guidelines.
- Proficiency in electronic medical records (EMR) and scheduling software.
- Exceptional interpersonal and communication skills, with the ability to build positive relationships with patients, families, and healthcare professionals.
- Compassionate, detail-oriented, and able to multitask efficiently in a fast-paced home health environment.
