Intake CoordinatorSaint Elizabeth Home Care | Warwick, RIFull Time | Days | On-SitePay: $22 - $25/hour (Based on experience)Our Intake Coordinator is a vital member of the clinical team who facilitates timely and accurate onboarding of new patients. The Intake Coordinator requires attention to detail and effective professional communication with internal and external stakeholders and clinical staff. The Intake Coordinator works closely with the Intake Nurse to review, accept and process referrals, maintain productive working relationships with referral sources and provide outstanding customer service to patients.Responsibilities of an Intake Coordinator:Collaborates with community partners and internal team members to process new referrals and provides timely updates on pending referrals
Maintains accurate and up-to-date patient status information throughout the intake process
Coordinates with insurance providers, physician offices, and case managers to obtain necessary authorizations and documentation to confirm eligibility for home health services
Collects Intake/Admission referrals from outside sources via fax, telephone calls and CarePort. Under supervision of Intake Nurse and Clinical Management post acceptance, enters new patient information into designated software and updates records as needed
Verifies patient eligibility and benefits and obtain necessary prior authorizations through Medicare and other insurance carriers
Responsible for daily Welcome Calls to new referrals to review patients' demographics, verify insurance memberships, gather security information and confirm patients are agreeable to home health services
Maintains a thorough and working understanding of the skilled scheduling process
Responsible for skilled staff scheduling in the absence of the Skilled SchedulerOther duties as assigned
Qualifications for an Intake Coordinator : Minimum of high school diploma or GED.At least 3 years of experience in a healthcare or home health care setting, with a focus on patient intake, coordination or scheduling is preferred.Ability to work independently with minimal supervision.Possess high attention to detail and organizational skills.Possess excellent interpersonal and customer service skills.Knowledge of healthcare regulations and insurance guidelines.Proficient in accessing electronic health records and utilizing various software programs.What does Saint Elizabeth Community offer employees?Our holistic benefit options encompass your Health and Wellbeing, your individual and family needs, your professional growth and more by including:Competitive pay
